- 1099R – Retirement Income (rollovers included)
- 1099-MISC or NEC – from subcontracting/self employment jobs
- 1099G – Unemployment Income
- 1099-SSA – Social Security Statement
- 1099-INT – Interest Income
- 1099-DIV – Dividend Income
- 1099-C – Cancellation of Debt
- 1099-K – Credit Card/3rd Party Network Transactions
- 1099-S – Real Estate Transactions
- Year-End Tax Statements from all Investment/Brokerage Accounts
DEDUCTION/CREDIT RELATED:
- 1095-A – Marketplace Health Insurance
- 1098 – Mortgage Interest Statements from all properties (including lines of credit or 2nd mortgages)
- Property Tax Bills – from all properties not shown on Mortgage Interest Stmts
- 1098T – College Tuition Statement
- Charitable Donations – Separate Totals for Cash and Non-Cash Donations
- Medical Expenses – Paid Out of Pocket – Separate Totals for Medical, Dental, Vision, Prescriptions (NOTE: Only Need if Total Exp is more than 7.5% of Total Income)
- Day Care Credit
- Providers Name, Social Security/Federal Tax ID Number, Address, and Total Amount Paid for the Year
- IL School Registration Fees (NOTE: Only Need if OVER $250 per Family)
- School Name, Grade, Total Fee (Do NOT include sports or yearbook fees)
- Energy Efficient Home Purchases – Insulation, Windows, Exterior Doors, Furnace, Hot Water Heater, etc (Do NOT include appliances)
- Item(s) Purchased and Total Amount
- For Self-Employed/Subcontractor/Direct Sales – Total Expense Amounts by Category (material costs, office exp, cell phone, internet, rent, insurance, taxes, licenses, etc)
- For vehicle usage, need total mileage for the year (covers fuel&maintenance)
- For Rental Properties – Total Rent Collected for the year AND Total Expense Amounts by Category (management fees, mortgage interest, property taxes, HOA fees, repairs, outside services, utilities, supplies, mileage to/from to check on property)
